Students will receive by mail the required forms: emergency medical forms, a student insurance purchase form from Stidham Insurance Agency, student insurance waiver form, field trip permission form, safety glass law form, and the Internet User Guidelines and Agreement. These forms must be completed and returned to the office secretary when the student schedule is picked up. No schedules will be given out until such time as all the forms are returned. Students will be notifed of schedule pick-up dates.

The Career Center requires all students to either purchase school student accident insurance through Stidham Insurance Agency or return the properly completed waiver form. This process must be completed prior to the student participating in assigned laboratory activities. A student not completing this requirement will not receive credit for lab until such time as proper documentation is on file with the Career Center.

Parents/guardians who wish to limit the distribution of their student's information such as name, address, phone number, birthdate, photo, etc. can download the Family Educational Rights & Privacy Act (FERPA) form at the right and return it to the principal's office at ACWHCC.
